دراسة مخبرية لحساسية العزلات المأخوذة من مسحات متنوعة لحمض الفوسيديك

الملخص

يُعد حمض الفوسيديك علاجاً شائعاً للعدوى بالمكورات العنقودية في ليبيا. وتُعد المكورات العنقودية الذهبية والمكورات الرئوية المسببات الأكثر شيوعاً لالتهابات الجلد القيحية السطحية (بنسبة 77% و2.2% على التوالي)، إلا أن التقارير أشارت إلى وجود معدلات مرتفعة للمقاومة بين العزلات السريرية. وتزداد عملية اختبار حساسية المكورات العنقودية الذهبية لحمض الفوسيديك تعقيداً بسبب غياب التوافق حول قيم الحد الأدنى للتركيز المثبط ومعايير الانتشار القرصي  المعتمدة لتحديد المقاومة. هدفت هذه الدراسة إلى تحديد العلاقة بين نتائج اختبار الانتشار القرصي ونتائج اختبار "E-test" (لتحديد الحد الأدنى للتركيز المثبط) فيما يتعلق بالحساسية لحمض الفوسيديك في عزلات سريرية للمكورات العنقودية الذهبية تم الحصول عليها من مختبر "السليم" الطبي. وقد لوحظ أن معدل الإصابة (بناءً على المسحات المأخوذة) كان أعلى لدى الذكور مقارنة بالإناث. وكانت البكتيريا الأكثر شيوعاً هي المكورات العنقودية الذهبية (57 عزلة، أي 77%) والمكورات الرئوية (9 عزلات، أي 12.2%). وأظهرت معظم السلالات المعزولة نسبة عالية من الحساسية المتوسطة  تجاه حمض الفوسيديك. وكشف اختبار الحساسية أن 4% فقط من عزلات المكورات العنقودية الذهبية و1% من عزلات المكورات الرئوية كانت حساسة لحمض الفوسيديك. وأظهرت نتائج الدراسة انخفاضاً في حساسية المكورات موجبة الغرام لحمض الفوسيديك في الاختبارات المخبرية؛ ولذلك، لا يُنصح باستخدام حمض الفوسيديك كعلاج تجريبي.
