الملخص

الخلفية: تُعد بكتيريا المكورات العنقودية الذهبية ممرضاً رئيسياً تتزايد أهميته نظراً لارتفاع معدلات مقاومة المضادات الحيوية، كما أنها سبب رئيسي للعدوى المكتسبة داخل المستشفيات. الهدف: هدفت هذه الدراسة إلى تحديد معدل الانتشار ونمط الحساسية للمضادات الحيوية لدى بكتيريا المكورات العنقودية الذهبية المعزولة من مرضى في مستشفى الجلاء للحوادث والجراحة في بنغازي، ليبيا. الطرق: تم اختيار 108 عينات عشوائياً من أقسام مختلفة ومن العيادات الخارجية، وشملت العينات مرضى من الجنسين ومن فئات عمرية متنوعة. تم الحصول على العزلات البكتيرية من مصادر مختلفة (البول، المسحات، أطراف القسطرة، أنابيب التنفس الاصطناعي، والدم). أُجريت عمليات تحديد الهوية البكتيرية واختبار الحساسية باستخدام نظام "BD Phoenix" (شركة BD Diagnostics، سباركس، ماريلاند، الولايات المتحدة الأمريكية). أُجريت هذه الدراسة الوصفية المقطعية في مختبر الأحياء الدقيقة السريرية في الفترة من يناير 2023 إلى أبريل 2023، حيث تمت عمليات الاستنبات والحضانة الهوائية عند درجة حرارة 37 درجة مئوية وفقاً للإجراءات القياسية المتبعة في علم الأحياء الدقيقة. خضعت العينات التي أظهرت نمواً بكتيرياً ملحوظاً لعمليات تحديد الهوية بناءً على مورفولوجيا المستعمرات، وصبغة غرام، واختبار الكاتالاز، واختبار التخثر (الكواغولاز). كما أُجري اختبار الحساسية للمضادات الحيوية (AST) باستخدام طريقة "كيربي-باور" لانتشار الأقراص. استُخدم برنامج "IBM SPSS" (الإصدار 28) لتحليل البيانات، واُستخدم اختبار "مربع كاي" لتحديد الفروق بين متغيرات الدراسة، مع اعتبار القيمة الاحتمالية (P < 0.05) دالة إحصائياً. النتائج: أظهرت النتائج أن المكورات العنقودية الذهبية كانت مقاومة لغالبية المضادات الحيوية؛ حيث لوحظت مقاومة لدى معظم المرضى تجاه البنسلين (104 حالات)، يليه السيبروفلوكساسين (103 حالات)، ثم السيفيبيم (102 حالة)، وحمض الناليديكسيك والسفترياكسون (101 حالة)، وتيكاريسيلين-حمض الكلافولانيك (100 حالة)، والنيتروفورانتوين (99 حالة)، والإيميبينيم والسيفالوثين (98 حالة)، والبيبراسيلين (96 حالة)، بينما لوحظت مقاومة متوسطة تجاه الأموكسيسيلين (56 حالة). وفي المقابل، وُجد مضاد حيوي واحد فقط (الكليندامايسين) أظهرت البكتيريا حساسية عالية تجاهه مع معدل مقاومة منخفض. الخلاصة:
تُظهر المكورات العنقودية مقاومةً لغالبية المضادات الحيوية المستخدمة في اختبارات الحساسية، وهي تلعب دوراً رئيسياً في حالات العدوى داخل المستشفيات وتُعد مشكلة صحية كبيرة. كما أن الاستخدام غير الرشيد للمضادات الحيوية واسعة الطيف -الذي يتزايد في غياب خطط مدروسة أو آليات مراقبة كافية أو تحديد دقيق للمضادات الحيوية المناسبة أثناء مراحل العلاج ومكافحة العدوى- يؤدي إلى فشل العملية العلاجية. لذا، ثمة حاجة لمزيد من التقييم والدراسة لبحث مقاومة المكورات العنقودية للمضادات الحيوية، ولا سيما سلالات المكورات العنقودية الذهبية المقاومة للميثيسيلين (MRSA).
